Health Homes Care Management 


In collaboration with Neighborhood Center and Center for Family Life and Recovery, care management services are provided to the seriously and persistently mentally ill in Oneida County

Services Include:

Advocacy

Linkages to mental health and physical health providers

Assistance in accessing benefits

Assistance in housing options


Funding sources for this project include the Oneida County Office of Mental Health & Medicaid.
